Brazil

Step 1: Fill out the passport form https://formulario-mre.serpro.gov.br/sci/pages/web/ui/#/passaporte-visto
Click on “passport”. It is not necessary to fill in all fields. If any requested information is missing, check the box “unknown”.

Step 2: After completing the travel document form, access the E-Consular platform: https://ec-saofrancisco.itamaraty.gov.br/
You must create a new registration. Registration can be done with your email address or CPF.
Click on “new service” > “Passaporte para maiores de 18 anos" ou "Passaporte para menores de 18” > “Start service”. Fill in all the information requested by the system and digitally send images of the documents and USPS money order.
The E-Consular website is a preprocess platform of passport applications, which will only be finalized after the Consulate receives the documents by mail or in person appointment. Requests received without having been previously validated through the e-Consular system will not be processed within the established deadlines and may be returned without processing.

Step 3: After receiving the validation e-mail from the E-Consular platform confirming that all documents are correct, please mail your passport application and documents in a USPS envelope. Don't forget to send a prepaid return envelope. Accepted envelopes: USPS Priority Mail or Priority Mail Express. If you wish to apply for your passport in person, click on the link provided in the E-Consular validation email to schedule your appointment. Processing time for passports requested by mail or in person: 20 business days.

- Police report is only required when the passport has been stolen – a french loss/stolen report will be created at the consulate
- 1 recent (< 6 months) passport size picture is enough for Laissez-Passer and Emergency Passport
- The emergency passport is issued for french citizen when your return flight doesn’t land in France
- The emergency passport are issued the same day
- A copy of the lost/stolen passport is useful as proof of citizenship – have a picture on your phone
- A proof of address is also required

Germany

Instructions
All German citizens should take an electronic copy (and share it with trusted others) of their travel document. If/When a passport is lost, the German citizen should contact the closest diplomatic office, likely the San Francisco Consulate General. The German citizen will need to apply for replacement/emergency/temporary (depending on the situation) documents in person, regardless of the situation.

Documents necessary for new documents are listed here:
- Completed application form
- two identical biometric recent passport photos (Please do not cut the photos yourself!) your birth certificate stating the exact place of birth (it is not sufficient if only the county is mentioned on the US birth certificate, a “proof of birth letter” issued by the hospital with the name of the town must then also be provided)
- if married, divorced or widowed: marriage certificate, if applicable divorce decree or death certificate of the spouse
- previous passport (data page with photo) Non-US-Citizens: valid residence permit in the US (US Resident Alien Card, US residence visa) and passport - Information for holders of a greencard without expiration date
- US-Residents: Proof of Acquisition of US-Citizenship (e. g. US birth-certificate, Consular Report of Birth Abroad, US Certificate of Naturalization with retention permit) and US-passport
- if the last name in your previous passport is not the same as the last name you currently use, a name declaration might have to be done. Please consider the Information on Name Declaration
- Proof of current name usage (i.e. one of the following: German birth certificate, German marriage certificate or if married outside of Germany, certificate of current name usage issued by the registrar’s office in Germany)
- if applicable, German Naturalization Certificate
- if applicable, US Naturalization Certificate with “Beibehaltungsgenehmigung” your US driver's license or an utility bill in your name as proof of residence
- if applicable, deregistration of your last place of residence in Germany (“Abmeldebescheinigung”), if it is entered in your last passport if you wish to have your doctoral degree entered in your passport, proof of residency and your old passport or German ID card with entry of your doctoral degree must be presented. If the doctoral degree is to be entered in the passport for the first time, your doctoral degree certificate must be submitted (the certificate should contain at least your first and last name, as well as your date of birth).
- Only for adult first time applicants: If one of your parents has had German citizenship at the time of your birth and if you therefore have obtained German citizenship by birth and not by naturalization, we need the following additional documents:
-- Passports of both of your parents (data page with a photo)
-- In case one parent is a US citizen without a passport: That parent’s driver’s license or State ID
-- Valid US residence title of the German parent (US Resident Alien Card or US visa)
-- Parents’ marriage certificate
- Passport fee
In addition, some German missions in the United States require that a stamped envelope for mailing your documents already be submitted at the time of application. The missions abroad that have this requirement will inform you of this in your appointment booking confirmation.

Apply online for a replacement
- You can report your passport as lost/stolen when you apply for your new passport.
- Tell us the passport is lost, stolen or damaged in the online application.
Apply by paper application form for a replacement
- Fill out Section 6 of the application form and explain how your passport was lost, stolen or damaged.
- Get Section 6 witnessed and stamped.
If you do not intend to apply for a new passport and want to report a lost/stolen passport
- You will need to report your passport as lost/stolen to the Gardai and submit a Garda certificate of loss along with a letter signed by you, requesting that the passport be marked as lost/stolen.
- You may be contacted for confirmation that you wish to have the passport marked lost/stolen, and for any outstanding information. You can post this to the address below:

Important details regarding lost/stolen passports
We report the details to Interpol.
The passport is no longer valid for travel.
If you report a passport as lost or stolen and subsequently locate it, do not travel on it.
We recommend you let the Gardai know to protect yourself from identity fraud.
We may restrict the validity of your next passport. Losing two or more passports may mean you do not get a full 10 year passport. You may get a new passport valid for somewhere between 1 and 10 years. This will depend on how many passports you have lost or had stolen over the last 10 years.
It may take longer to process the application for your next passport.

In order to apply for a new passport or travel document, you must visit the Consulate in person.
If your passport or laissez-passer/travel document is lost or stolen, you must report it to the local police immediately.

South Africa

Instructions
Currently, the New York office is the only one that can help with Lost Passports for South Africans who live in South Africa and have traveled to the USA to come to Burning Man. They are able to issue an Emergency Travel Certificate, which allows for one way travel into SA.

The South African Consulate-General is open Monday to Friday from 8:00 am to 4:30 pm. Consular hours (Visa & Passport) are from 9:00 am to 12:00 noon, excluding public holidays.
Authentication of documents takes place Thursdays and Fridays between the hours of 9:00 am - 12:00 noon.

Switzerland

Instructions
The loss of theft of a Swiss ID document abroad must be reported as soon as possible to the local police as well as to the relevant Swiss representation.
A police report (declaration of loss) is also required to make it possible to prepare a new identity document, such as a temporary passport. Some countries may require such a document in order to leave their territory. It can also be useful when seeking reimbursement from an insurance company.
Swiss passports / identity cards which have been reported lost or stolen to a Swiss representation are entered in a Federak Office of Police (fedpol) file, and if subsequently recovered must not be used in an ID check or for travel, to avoid future difficulties. They are to be quickly returned to a Swiss authority for destruction.
